Contact form in profile edit always blank was created by CCANSW
I have enabled "Edit contact in profile"

However, when a linked user for a contact opens their profile for editing, the contact form appears empty.

What might I have missed setting this up?

Did you select the fields you want the user to be able to edit in the plugin 'Edit contact in profile' ?

Does "User - Contact Creator" need to be enabled? (I'd prefer not)

No, 'contact creator' is not required, unless it's a new user and there is no contact associated with him/her. That plugin is to create a contact alongside a user and links them automatically when a new user creates an account on your site.

Make sure the user who edits his/her user profile is linked to a contact. If not, nothing will happen.
